  1. If grade B in a grad course if bad then what is a grade less than A in any undergrad course? Catastrophic, I bet. Do undergraduates freak out their B's? I can't even imagine.
  2. Remember the good old saying: "it will never happen to me"? It's pretty much true. Or is it?
  3. Anything is possible if you're willing to do the hard work. I know because I came back from the dead myself, so to speak. You may have to rebuild your academic history a little bit, kind of like a lender may rebuild his or her credit history before applying for a loan. Take some courses related to the program you want to apply for (maybe some missing prerequisites for example), if you are eligible and do well in them. Your most recent academic history is the first thing schools will look at (from my experience). Do it for yourself. Regain your confidence and prove that you're willing to work hard... to you yourself. The rest will follow. Good luck.
  4. I'm not sure what computer science courses require more than calculus or discrete math. Basic linear algebra may be useful for a graphics-related course. Other than that, once you're in the program, I would image that you will be able to pick whatever math you're interested in, depending on your goals. Numerical Analysis - that's a really good one. I have yet to take this course myself. Number Theory - theory of congruences, prime numbers are useful in Cryptography, hashing, etc.
