Forgot to list school/transcript on application and have been recommended for acceptance

I submitted an application for admissions to a Ph.D. program, and upon checking the copy of my application online, I discovered that I didn't add the school where I received my MA. I added my graduate GPA but apparently did not add the school or the transcripts on the application interface. The school was mentioned in my SOP as well. This was a complete oversight on my part. I know that I will eventually have to send over the transcripts, but I'm wondering if I should do that now, or wait for them to contact me? (if they notice) I haven't received my official acceptance letter yet, though I've been recommended. I don't want them to assume I purposely left the school out, or mark my app as incomplete. Do you think this will affect their final decision? Has this happened to anyone else before? What should I do?
