GRE Optional (to submit or not submit)

I am applying to biomedical Science PhD programs at the top of the field.  While my application is good, my GRE scores are just okay. 

Verbal 165 (96)

Quant 158 (68)

Writing 4.5 (82)

Most schools at the top list GREs as optional.  So, if I have a stellar application will submitting my GREs help or hurt?

and aside from that, for non-optional programs do I need to retake?
