Grad School CV

What should one include in a CV that will be attached to grad school apps? My understanding is as follows: include honors/distinctions, published articles, conference participation, and relevant work/research experience.

Am I missing anything?

If that info fills up enough space, I think you covered the important parts (obviously, in addition to where/when you graduated, degree info, contact info, etc). I'm also listing relevant courses for a few classes whose content can't be understood with quick reference to the transcript (i.e. certain seminars and two grad courses)

Good call about the courses. I need to get a copy of my transcript to understand better the way that class/seminar info is conveyed. Is length or filling up space something to seriously consider here?

I would say no, unless you're having trouble with white space on the first page.
