LOR: Academic vs. Work

I'm a bit over 3 years out of undergrad (graduated in 08) and have worked at the same company since then. Right now I have my current supervisor writing a recommendation, and am going to have an old professor do one as well. For the 3rd LOR.. should it be professional or academic? Would I be better off choosing another professor, or another colleague? I have also been involved in a few 'extracurriculars' over the years since undergrad, and have had supervisors through those, but not any whom I was super close to. Thoughts? Recommendations?
