Former professor no longer in academe

My former professor is no longer teaching. On that section where they ask you for your recommender's info/position, should I enter my professor's former title and school where I studied, or do I enter his current title, e.g. Joe Smith, Phd. Manager, XX Company.?

I was in a similar position and my former prof said to use her current title, and she explained our advisor/student relationship in the letter. Maybe ask your referee what he thinks?

Thanks for the reply. I asked him but he said wasn't sure how I should do it. He will be saying something about our relationship in the letter though.
