Submitting a published article-formatting questions

Hello!

 

I'm applying for Art History PhD programs, and my chosen writing sample is a paper published in a peer-review journal a few years back.  I have a few concerns about the format typically expected for writing samples, and would really appreciate some advice!

 

Following are my issues:

 

(1). My article was proofed using PDFs and snail-mailed edits, so I only have PDFs of the final version of the article.  The only Word doc I have of the article is probably the first draft of the piece before we began the editing process.  

 

(2).  The final document I have is formatted single-spaced, with two columns per page and footnotes; I was not required to submit a works cited to be included with the published article. The document also includes the Table of Contents for the Journal, which I am not sure whether I should remove (the TOC has the journal title and number at the top - I'm not sure how else to indicate the submission is a published article).  I don't have an issue with adding a page at the end with works cited, as I'm pretty sure this is possible with Adobe Acrobat.  Should I delete or keep the TOC, and add or not add the Works Cited?  Will the adcomm take issue with the format of my writing sample being journal-style and not that of a traditional double-spaced coursework essay?

 

If I absolutely have to, I can go back through the Word document and the PDF side by side, and make changes to the Word doc so the content matches the completed article.  However, for time's sake I would highly prefer not to do this if it's unnecessary.  I actually like the visual impact of the journal format, but I don't know that I can make edits to the body text in a PDF format.  What do you think?
