transcript question

Hello,

 

I have question about upload transcript on online application. 

I have gotten official pdf downloaded from my college and graduate school application requires scanned copies of official transcript. 

Is that mean, that I actually have to have printed official transcript and scan them for upload on to online application? 

Also, I have took one graduate course from one institution after i graduate from college, and i got C on that class. 

Should i include to my CV and send it for graduate school application?
