  1. Have you done any research? Any published, or at least submitted work? I think GPA and GRE scores, however good, will only get you through the first filter.
  2. I would think that for a top program anything below 90% on math is not good enough.
  3. Probably the best way to approach professors is having studied their research and suggesting some contributions to their work, not too involved, but that at least show your interest and potential. E-mails that are too generic are usually ignored. Even though the committee makes the decisions, individual professors are usually consulted about their preferences. I really don't think there's such thing as a "good" time to write to professors, as long as you don't do it too late into the application process. Sergio
  4. If you want to get into a Top 5, it will be your research work that makes the difference, not your grades. If you already interviewed, that means your grades are OK
