The fellowship application is within the admissions application (through UCLA Graduate Division). You will check the box for the Graduate Opportunity Fellowship and then follow the subsequent instructions to upload a statement, etc.

The fellowship app is part of the grad application system. If you are applying for one of the diversity fellowships, a supplemental section will appear once you select the fellowship(s) you want to be considered for.
I have a question about the CSU mentor application. I'm applying to a few CSUs and on the applications I've looked at, there's a section that asks for three academic references. I am planning to use two professional references and one academic reference, since the departments do not require three academic references. Does anyone know if we have to fill out this section or can we write something along the lines of "sent directly to the department"? I know that's usually the case for the statement of purpose.
