Hope it's not too late to contribute, but University of Memphis, University of Utah, and University of Washington have Clinical Health tracks. Virginia Commonwealth University has a Ph.D. in Health Psychology. I'm interested in Clinical Health psychology too, so maybe we can help each other out!
