What are recommenders asked other than LoRs?

My understanding is that they receive emails to submit LoRs. But along with the LoRs, I heard they are asked several questions, such as how do you know the applicant; how long have you known the applicant, etc. What exactly do they ask to the recommenders and what should I tell them (some are not professors and have no experience in writing LoRs) to expect? Are they also given an evaluation form –often a grid assessing your leadership traits, etc. – like MBA schools do?

Still can't find any clues. I am sure there are people who have applied last year and have gone through the recommendation process. Could anyone possibly answer this question?
