Getting around TOEFL

Hey fellow grads/grad school aspirants,

I have a question about how to bypass the TOEFL requirement if all your schooling was done with English as the medium of instruction. Most schools have a note that they will waive the TOEFL requirement for PhD programs if your language of instruction in Undergrad was English. Have any of you tried to do this? What did you have to submit to show them that your instruction was in English?
