I made a single application to Chicago Harris last year at the last minute. I didnt take the GRE and thus my application wasnt complete, as I had found a great internship and felt my time would be better served doing that.

My question is, I'm applying again this year and was wondering how I should play this. I sent the application in but it was never read or considered. Should I mark down I sent in an application last year or not?

If you paid the application fee you applied. If you did not pay the fee you did not apply. Whether you sent all of the required support for your application after you applied isn't the question they're asking.

I paid the fee but they didnt consider my application. I was wondering more whether

1) I need to check I applied before

2) Address it in my SOP

If you paid the application fee you applied, so you would check the "yes" box. You do not need to address it in your SoP; doing so would waste precious space where you could be discussing something that matters.
