I got an email from my supervisor saying he and the grad chair had signed off on my app and now it just has to go to the Grad centre. Is there any chance they would block it if the dept is behind me?

Not really. That final check at the Grad Office is just for official formalities, paperwork etc. Barring exceptional circumstances, there is no reason for them to block it.
