Yeah, your name is usually on the application. Personally, I wouldn't worry too much about if that person is on the committee- I think it mostly boils down to your hard stats meeting their cut-off (GPA, GRE, etc.) and the prof who wants you in his lab. I feel lke if you have the stats and a prof wants you, there's not much someone else on the committee can do to block it (maybe I'm wrong, though?).
