Are you allowed to defer a year if you got a fellowship?....and keep the fellowship?

What about the idea of going a year, then taking a leave of absence a year? Can you get it back? What if the reason is medical in nature?

 

I know these cases are all individual, but there must be a standard protocol that schools generally follow.
