Wrong DOB and last name in GRE registration, test in Nov, can it be solved? :(

Ok so I was just reading through my appointment and realized the DOB mistake first, entered the month wrongly, June instead of July.

 

Later, I realized that I used the wrong Last name, my complete name has 4 names, last one being the family name (so I entered that one) but my passport shows the surname as my dad's name (the second name) i didn't realize it earlier and I'm PANICKING now! 

 

has anyone ever come across such an issue? was ETS helpful to solve it?
