Looking for Personal Statement Critique

I'm applying to PhD programs in public health this coming December to start in fall 2017. My focus is public mental health and my research interests include health service access and health policy. I'm transitioning from a master's education in counseling and have been involved in a couple of related research projects. I'm posting to get help with constructive feedback on my personal statement. Would appreciate the help from someone who has completed or is completing a PhD in public health. Can also give mutual feedback if you are applying like me. If you are or you know of some one who is going into public mental health, I would be thankful if they can help!
