"Please give the names and professional affiliations of individuals"

I'm applying to uchicago and the app has a section stating "Please give the names and professional affiliations of individuals most responsible for your decision to apply to the University of Chicago." I can't tell if it's asking for professors at the institution whose work I am interested in or asking for a list of (outside?) people who have influenced me to apply. I'd email the program for clarification but it's a little late for that--due today.

No clue! That might be the sort of question where you should email someone in the department, maybe not your POI, but someone in administration.

I'm applying to UChicago too and had the same problem figuring out what they mean. Mine's due the 10th, but I still can't bring myself to really think it's worth emailing the program or school about. I left it blank. Seems more like a survey question anyways, not something super important.
