Question about Signing the I-20 Form

Hello,

 

I need to sign the I-20 before the visa interview , I am above 18 years old. I am kinda confused about these two things :

1)Do I need to put my address?, I am not sure because the address field is inline with the part about the guardian.

2)There are 2 fields for date in which one should I put the date?

I think the second date goes with the parent's sign. You'll just need to provide your city, state (if applicable), and country in that line.
