Course Justification Request Explanation

Hello, I have to submit a course justification request and write a brief explanation/description. I would like to know if anyone here have an example of the letter. I am not sure how to write it. Thank you.

Additional information:

I am accepted to a PhD program. There are some courses I took in my previous degree qualified to be transferred to my new program so I can save my time and money. However, I have taken those courses long time ago, so I need to submit the course justification request to show that I am working on the field and have been using the knowledge I gained from those courses. I have to submit the form and explanation letter to grad school chair and committee.

To whom is the justification being submitted? For what purpose? Without this additional information, it will be difficult for anyone here to help you.
