Is anyone still waiting for the actual graduate school to send them the official offer? I was informed by the program of my admission with an email and phone call but have yet to receive the offer from the grad school. They did say they don’t release offers until the program gives them the official list of offers. Does any other school do this? 

It’s just making me nervous.

I'm not sure if I'm getting at exactly what you mean, but I accepted to a school over a week ago and still haven't received anything asking me to make the deposit for my spot. I'm sure it's just a busy time and such, but I wouldn't start worrying about it yet! :) Or at least that's what I'm telling myself :P

I don't think it's anything to worry about. Similarly, the school that I accepted the offer to gave me a formal acceptance from the department, but the graduate school let me know that they will send the official acceptance after the check my official transcript with my degree. I graduate in May, so I will have to wait until then to receive this official final admissions, but the department told me it's nothing to worry about, just a formality. It sounds like you might be in a similar situation :)

I'm pretty sure that the program has to transfer all the applicants over that they have accepted to other departments to create log in, email and student id information. Then this will be sent out in the mail. That's what happened with my program and then this gave me information on how I submit my deposit and what not.
