It seems like some schools send out "initial" notifications in February, then follow-up with confirmation and/or funding offers in March. But I think it all depends on the program. I can only apply to Ohio State University because my husband is established in his career and moving would be very difficult. I have been in regular contact with a professor, and apparently I am #1 on the wait list. Only 4 PhD students are accepted per year, and all are fully funded.
