Long story short, there is a possibility that I may graduate over the summer instead of this spring as planned. I have already been accepted into a graduate school program for the fall and they are requesting I send a final transcript indicating my degree, but I won't be able to send it until August instead of June. Could the graduate school rescind my offer because of this?

They could, but I doubt they will. Its best to be honest and ask them. We really can't help you.

You need to contact them and ask. But as long as you have completed all your courses before the start date of your grad program (and can prove it) it shouldn't be an issue.

Call them.  But, I doubt it.  I'm in the same position and I'm going to defer for a term (Fall 2015 instead of Summer 2015).  It's really not a big deal.  I almost had a heart attack before calling my admissions office about this, and they completely allayed my fears.

Usually you have to have your undergrad institution send an official letter of graduation intent with expected degree date--I had to do that when during my senior summary in my last semester it was discovered I transferred too many courses and did not have half of my hours from my BS granting institution. I had to enroll in 12 hours of anything to graduate so I took 12 hours of random easy load courses and my degree date changed to end of July. It wasn't an issue.

You should ask but, in my experience, their concern is that you have graduated before you start your graduate coursework. I moved my MA graduation from May to August and no one even asked me about it.
