From my experience pubs are not a necessity but they are a huge help. They show to the adcoms that you're serious about research and have experience doing academic work. I don't imagine there being many first authors among applicants.

It depends on what you want to study in management. Some research will require a heavier background in math (e.g., finance), while others will be more psychology or sociology (e.g., OB). I'm not sure where entrepreneurship or international business fall, but a lot of schools generally lump those with OB departments so I assume they're more social science. If you're more interested in the social science part of business I would recommend stats classes over math. Math classes will give you a better foundation for stats but could be overkill for the level of stats you'll need to be successful. With what you shared you should be fine getting into a decent program. I would recommend not emphasizing your desire to teach too much in your applications. The goal of most PhD programs is to develop researchers. There are a few programs out there that have classes on teaching (U Washington comes to mind) but they are rare.

Once signed in a box appeared about half way down the right side of the page that said "Who's Viewed Your Profile? 6 Your profile has been viewed by 6 people in the past 30 days." I followed the link and could see who's viewed my profile. I couldn't see the exact person from each school but I was given a list of about 10 people that could have accessed my profile. The odd one was Florida. I didn't apply to their program but someone from the department viewed my profile. It's not an exact science but it's something else to check.

I thought we were supposed to hear from UNC last Friday… I'm beginning to think that since I haven't heard anything that means I'm not on the interview list. Depending on how you interpret "end of the second week in February" we may not hear anything until this Friday (the end of the second full week in February). I'm still really grateful that they let us know about the first cut; the other programs I applied to don't tell you anything until you get interviewed or rejected.

My impression is that every school has a lower limit of what they'll consider with the GMAT. If you pass that threshold they'll look at the rest of your application. The top 10-15 schools are probably around 750, top 30 probably around 700, and top 50 schools are probably 650 (these are just my best guesses).
