  1. If websites say that you must name a dapartment you are applying to - absolutely do that. Otherwise it is not necessary. I put in my header the following info: "My name - Statement of purpose - PhD program in blablablology at the Great University". It is not necessary to include the last part, though. It does not matter. Websites usually mention which formats you can upload and PDF and WORD were fine for all schools that I have seen so far. There is nothing unacceptable or unprofesional in any of these formats

  4. I agree with Modernity. The best writing sample is the one on on the topic of research that you want to do in the program. But... Will you have time to finish it? When is your deadline? It is difficlt to write a really good article in just a couple of weeks, unless you are an expert

  9. I think they will understand. But you should contact them and explain the situation. Also contact your recommender and say very politely that the letter to Stanford must be uploaded asap.

  12. Even if it is not mentioned on their websites, you are supposed to send transcripts in envelopes to every school. Otherwise they are not considered official. So, unfortunately, you will have to resend all your transcripts...

  15. Do you have an account at the official GRE website? You can see there the list of schools you've sent your scores to and the day they have been sent.

  18. If you are requested to submit 3 LORs and you WILL submit only 3 LORs, there is no reason why you should write that you will submit 4 LORs on your application. It may confuse adcoms. Perhaps they will be waiting for that fourth LOR to arrive. If you indicate only three recommenders and your fourth recommender sends his/her letter in the end, it will not be a problem. But if you indicate four recommenders and one won't send a letter, it can bit a (little) problem. Why do you want to sumbit 4 letters for programs that request only 3 anyway?
  19. I am sure you can sumbit a new one. Just contact a grad coordinator and don't worry. As the others have said, we all have made minor mistakes in our apps. I found some myself. First I was nervous but then a friend told that if a school is going to reject me because of a couple of tiny mistakes - this school just is not worth worrying about
  20. That is mean! One of my schools also changed their deadline in November from 10th December to 1st December. Luckily I was going to turn my app in earlier anyways. What are they thinking about when they do such things??
  21. I agree with Bhikhaari. If you skip this question on the application form but discuss your work experience in a SOP and also send a CV or a resume, it will be just fine. Do not worry about that
