Yes you do. It doesn't provide a lot of information beyond a score though (I was forwarded from my institution a couple of years ago, but waitlisted with a "decent' score). It's a score out of 20. Some will depend on the committee that is evaluating though. For instance, there were some people with 13/20 that were waitlisted, and others that scored 14/20 and got the bombardier award.
My understanding (from a faculty member who has sometimes evaluates these for SSHRC) is that there is an enormous amount of subjectivity, and one hopes one doesn't have a "grouch" evaluating your application that day.
